In the Ombudsman’s words

allowed payments to be taken from her account and didn’t cancel them when she askedUnauthorised transaction
Miss P complained to us. Our investigator could see the payments taken hadn’t helped Miss P’s financial position but as she was already overdrawn she didn’t think the problems she experienced were solely due to the mistake by Lloyds. This page summarises the Ombudsman’s published final decision. The document itself is the record: DRN6119857.pdf on financial-ombudsman.org.uk.

The outcome shown is the Ombudsman’s own published classification, not our reading of the text. Nothing here is legal advice, and a decision concerns only the complaint it was made about.
