Lloyds replaced Mr M’s debit card in February 2018. Mr M was unhappy at having to contact Lloyds after he found a new PIN was required to operate the card, which he’d not been told at the time the replacement was issued. And he was dissatisfied with the delays in issuing him with the PIN.Service failure
